Life and Health

The Life and Health Division works directly with insurers and insurance company product related filings. The Life and Health Division also contains a Utilization Review Section and a Managed Care Section. These two Sections deal with managed care utilization review, help resolve medical provider and payor claims issues and conduct educational and outreach programs regarding appropriate claims filing and paying practices for health care providers.

The Division’s responsibilities include, but are not limited to analysis of life insurance, annuity, credit insurance and health insurance policy forms, review and approval of new or revised premium rates for certain categories of health insurance or credit insurance products and advertisements for Long Term Care and Medicare Supplement Insurance as required by laws or regulations, and certification of managed care insurers under the Patient Protection Act.

Effective August 30, 2010, the Office of Insurance and Safety Fire, Life and Health Division, changed the address for mailing in filings with payments. The new payment mailing address is:
